ACBR Part II Examination Application 2025

Thank you for your interest in the ACBR Part II Examination. Carefully review the Candidate's Guide to confirm eligibility to sit for the examination. Applications are due May 31, 2025 by 11:59 PM EST. Applications and examination fees must be received prior to the due date to be eligible to sit for this year's examination. Following closure of the online application, the Exam Coordinator will contact candidates via email with instructions for accessing SharePoint and uploading candidate imaging portfolios. List the name of the chiropractic institution sponsoring your radiology residency. If the institution has multiple campuses, please specify campus location. List the start date of radiology residency (month, year). List the anticipated graduation date of radiology residency (month, year). Upload verification of residency contract or employment from your institution HR department (see Candidate Guide for details). Include the country and state (if applicable) of current and active DC license/registration. If candidate maintains multiple licenses, document primary license only. Identify the status of current DC license/registration. If your license/registration has ever been on probation or you have faced any disciplinary action, list in detail below. If there is no restriction to disclose, state "none". Upload a copy of current and active DC license/registration (PDF or JPEG). License expiration date should be visible. List the year candidate passed Part I ACBR examination. Select the number of times you have taken the ACBR Part II Board exam. First Time Candidates Only: Upload a written letter of recommendation by your current residency director including all elements required (as published in the Candidate Guide).
